The Ensign Group Expands in Tennessee and Enters Alabama with First Facility

The Ensign Group, a parent organisation providing skilled nursing and senior living services alongside various rehabilitative and healthcare offerings, has announced its acquisition of nine skilled nursing facilities across Tennessee and Alabama.
The facilities acquired include:
- Meadowbrook Healthcare and Rehabilitation Center (83 beds) in Pulaski, Tennessee
- Wellpark Health and Rehabilitation (30 beds) in Knoxville, Tennessee
- Legacy Park Health and Rehabilitation (176 beds) in Knoxville, Tennessee
- VanAyer Senior Living and Rehabilitation (91 beds) in Martin, Tennessee
- Union City Health and Rehabilitation (115 beds) in Union City, Tennessee
- Decatur County Healthcare (125 beds) in Parsons, Tennessee
- Savannah Nursing and Rehabilitation (120 beds) in Savannah, Tennessee
- Westwood Nursing and Rehabilitation (90 beds) in Decaturville, Tennessee
- The Health Center at Research Park (91 beds) in Huntsville, Alabama
Six of these facilities are jointly owned by CareTrust REIT, Inc. (NYSE: CTRE) and a joint venture partner, with operations leased under a long-term triple net master lease effective from January 2025. The remaining three were purchased by Standard Bearer Healthcare REIT, Inc., a subsidiary of Ensign, effective December 2024.
This acquisition marks Ensign’s first operations in Alabama and strengthens its presence in Tennessee. The company highlighted that local leadership teams have been preparing extensively for this transition, aiming to integrate the facilities smoothly and enhance their performance within the organisation's network.
Ensign currently operates 333 healthcare facilities across 15 states, with 30 of these also offering senior living services. Its subsidiaries, including Standard Bearer, hold ownership of 129 real estate assets, of which 96 are operated by the company.
The organisation remains focused on expansion and is actively pursuing opportunities to acquire real estate and lease skilled nursing, senior living, and other healthcare-related businesses, including underperforming operations, across the United States.
