Sentara Health and HealthSnap Partner to Launch Enterprise Remote Patient Monitoring and Chronic Care Management Program

HealthSnap, a leading virtual care management company, today announced that Sentara Health, an integrated, not-for-profit health care delivery system, selected HealthSnap's Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) solutions to launch and scale its integrated care model for patients living with chronic conditions. Sentara joins the over 150 leading health systems and provider organizations leveraging HealthSnap's platform to help support their patients living with chronic diseases.
HealthSnap's technology enables care teams to proactively manage chronic conditions, helping patients stay connected to their care teams between clinic visits and significantly improving patient outcomes. The initial rollout of the partnership launched across 12 primary care sites in August 2025. Sentara plans to rapidly expand the program to reach approximately 389 primary care providers and over 85,000 eligible patients throughout its network, focusing on chronic condition management, particularly hypertension, diabetes, and heart failure, with plans to extend services for other conditions in the future.
