Ochsner Digital Medicine Partners with Hawaii-Based AlohaCare

Ochsner Digital Medicine is set to collaborate with AlohaCare, a Hawaii-based health plan, in delivering cutting-edge digital medicine services to its members. Commencing from September 1, 2023, this strategic alliance will enable AlohaCare members to enroll in digital programs, aimed at enhancing their overall health and well-being.

Founded in 1994, AlohaCare is a community-driven, non-profit health plan established by Hawaii's community health centers. The plan is dedicated to serving around 80,000 Medicaid and 2,300 Medicare members, focusing on empowering its members through wellness promotion and facilitating access to top-tier healthcare.

Under this innovative partnership, Ochsner Digital Medicine will extend remote programs to AlohaCare members diagnosed with Hypertension and Type 2 diabetes. These programs employ remote patient management techniques using digital devices and personalized plans, encompassing medication management, lifestyle adjustments, progress monitoring, and motivational support. This comprehensive approach will aid AlohaCare members in achieving healthier lives. Each member will be assigned a licensed clinician and a dedicated health coach, working in tandem with their primary care provider.
