Netsmart, a leading provider of healthcare information technology for community-based care providers, has teamed up with VNS Health, a prominent post-acute care service provider in New York. This collaboration aims to enhance end-of-life care using machine learning and predictive analytics.

The partnership focuses on improving patient visit scheduling, enriching the experiences of patients and their families, and aiming to meet quality measures for Hospice Visits in the Last Days of Life (HVLDL).

As one of the largest non-profit home and community-based healthcare organisations in the United States, VNS Health is committed to offering compassionate care for individuals requiring home care, especially those with serious illnesses.

This partnership will allow Netsmart clients and the wider hospice sector access to VNS Health’s proprietary predictive analytics for HVLDL, which have now been integrated into the Netsmart myUnity® electronic health record (EHR) platform. The ONC-certified myUnity solution merges various care settings into a single patient record and incorporates the tools necessary for value-based payment models into one platform.

VNS Health’s exclusive HVLDL algorithm analyses each hospice patient’s case history and identifies crucial clinical indicators over a seven-day period. This machine learning model has been trained on over 1.9 million health records from nearly 10,000 historical patients. It provides a data-driven approach to generate more accurate, evidence-based care recommendations, assisting providers in delivering timely care for hospice patients in their last days.

This initiative has already enabled VNS Health to surpass state and national averages, improving HVLDL quality measures by 61%.

The HVLDL project contributes to the expanding portfolio of augmented intelligence, machine learning, and robotic process automation within the Netsmart CareFabric® platform, facilitating greater automation in healthcare delivery. Netsmart's suite of AI and automation technologies is designed to enhance clinical decision-making and operational efficiency, allowing providers to offer more effective and personalised care.
