Blue Cross Blue Shield of Arizona (AZ Blue) has announced a new collaboration with Monogram Health, a leading provider of value-based care and benefit management services for individuals with chronic kidney and end-stage kidney disease. Through this partnership, AZ Blue aims to enhance the care experience for its members by tapping into Monogram's extensive network of in-home providers and support services.

As part of the agreement, Monogram Health will take on full financial responsibility for BCBSAZ's Health Choice Pathway D-SNP Product members who have been diagnosed with kidney disease. This unique initiative will grant residents in eight counties across Arizona access to Monogram's specialized clinical and support services, including evidence-based clinical pathways, in-home specialty care, complex case and disease management, and comprehensive medication therapy management.
