Modivcare Teams Up with Michigan Centre for Rural Health to Improve Access to Care Through Remote Patient Monitoring in Rural Hospitals

Modivcare, a technology-driven healthcare services company, has announced the successful conclusion of an 18-month Remote Patient Monitoring (RPM) pilot programme. This initiative, carried out in partnership with the Michigan Center for Rural Health (MCRH) and funded by the Michigan Health Endowment Fund, aimed to enhance healthcare delivery in three rural hospitals in Michigan.
The pilot was implemented in Helen Newberry Joy Hospital, McKenzie Health System, and Schoolcraft Memorial Hospital, all critical access facilities serving small communities. The programme focused on increasing digital health capabilities and improving access to care while gathering insights to share with rural health stakeholders across the state.
During the pilot, 117 patients diagnosed with conditions such as hypertension, diabetes, and congestive heart failure were referred to the programme, resulting in an enrolment rate of 48.72%. Patients referred by their primary care providers or local care managers had significantly higher enrolment rates compared to those contacted through letters or cold calls.
A total of 46 patients participated continuously for six months, allowing for thorough analysis of health outcomes. The results showed a substantial increase in patients achieving controlled blood pressure, rising from 39% at the beginning to 78% by the end of the pilot. Additionally, patients with previously uncontrolled hypertension saw an average reduction of 27 mmHg in systolic and 11 mmHg in diastolic blood pressure, with many also improving their blood glucose levels and weight.
The programme demonstrated that RPM is particularly beneficial for homebound patients or those with limited social support, highlighting its role in addressing healthcare gaps in rural areas. Despite some connectivity challenges, the use of cellular-enabled devices resulted in a device failure rate of under 10%.
The success of this pilot has encouraged two of the participating hospitals to continue offering RPM services. A comprehensive Remote Patient Monitoring Playbook has been developed to assist other healthcare organisations in implementing similar solutions to enhance patient care in rural and underserved areas.
The Michigan Center for Rural Health expressed gratitude to the participating hospitals for their dedication to the RPM pilot, which not only benefited local patients but also contributed to broader health improvements through the dissemination of the playbook.
