HealthPartners has unveiled an innovative hybrid preventive care program aimed at simplifying and making health screenings, immunizations, and other preventive healthcare services more accessible and cost-effective.

Andrea Walsh, President and CEO of HealthPartners, emphasized that while digital-only solutions are on the rise in the market, HealthPartners is taking a distinct approach. By combining the strengths of their health plan and healthcare delivery, they aim to bridge the gap between digital and in-person care, offering consumers the best possible experience and value. The introduction of hybrid preventive care represents their latest innovation in this regard.

Here's how the hybrid preventive care program functions:

  1. Utilizing a Comprehensive Healthcare Network: HealthPartners leverages its team of clinicians and a network of 50+ primary care clinics, known for their high performance in terms of quality and total cost of care. They also integrate their 24/7 online clinic, Virtuwell, into the program.

  2. Personalized Preventive Care Plans: The program employs clinicians, algorithms, and augmented intelligence to create individualized preventive care plans for each patient. Patients start by completing a digital smart assessment covering their current health status, medical history, mental health, and more.

  3. Expert Assessment: A nurse practitioner reviews the patient's assessment, along with electronic health data and claims information, to determine their specific preventive care needs. The nurse practitioner then recommends any necessary additional care, such as mammograms, colonoscopies, immunizations, or lab tests. Patients also have the option to receive counseling and coaching by phone.

  4. Seamless Access to Care: Through this hybrid experience, patients can easily schedule the most convenient appointments, whether in-person or virtual, for any follow-up care they may require. The program also provides clear definitions of what constitutes preventive care, helping patients better understand their health plan coverage.

Additionally, while the hybrid preventive care program is open to all, it is particularly optimized for individuals with HealthPartners plans, including the high-performing Select network. This can potentially lead to significant cost savings for employers, with the possibility of reducing premiums by up to 15% compared to broader network insurance options. The program is set to launch in January 2024, and HealthPartners intends to further expand this innovative hybrid care approach to cover other medical specialties in the future.
