BayCare Launches Connected Care Program to Bring Top-Quality Healthcare into Patients' Homes

BayCare has partnered with Cadence, a chronic disease management company, to launch a remote patient monitoring and virtual care program.

This collaboration will combine BayCare Medical Group’s clinical expertise with Cadence’s technology to deliver proactive care for seniors with chronic conditions across Florida’s Tampa Bay and West Central regions.

The program will focus on managing patients with hypertension, congestive heart failure, and type 2 diabetes. Patients will benefit from real-time monitoring of vital signs such as weight, blood pressure, heart rate, and glucose levels, which will be shared with their primary care physician and Cadence’s 24/7 clinical team.

Cadence will provide continuous monitoring, medication management, lifestyle guidance, and timely interventions if any vitals fall outside normal ranges, aiming to improve health outcomes.

The initiative reflects BayCare’s commitment to advancing patient care and enhancing the healthcare experience. It represents an effort to provide innovative, high-quality care for patients with chronic conditions and support healthier lifestyles.

Cadence’s nurse-practitioner led team currently supports over 25,000 patients and 1,200 providers across 22 states.

Their 24/7 monitoring and care have led to a 61% reduction in heart failure hospital readmissions, a 107% increase in patients achieving target blood pressure levels, and a 43% increase in those reaching target blood glucose levels.

This partnership underscores BayCare’s dedication to community-centered care. By integrating remote monitoring and chronic disease management, BayCare aims to enhance patient experiences and improve provider efficiency.
