Astrana Health, a prominent technology-focused healthcare company dedicated to enhancing provider delivery of accessible, high-quality care, has entered into a strategic partnership with Elation Health.

Elation Health, a leader in clinical technology with a robust electronic health record (EHR) platform, will join forces with Astrana to advance value-based care for primary care providers.

The partnership will focus on expanding risk-bearing entities, including accountable care organizations and clinically integrated networks, to support independent physicians.

Elation will contribute its acclaimed EHR platform, practice transformation tools, and billing solutions.

Astrana will provide its comprehensive value-based care platform, which includes value-based contracting, credentialing, AI-driven population health analytics, an NCQA®-certified HEDIS gaps in care engine, care and disease management tools, and various administrative services such as billing, coding, and call center support.

To showcase this collaboration, Astrana will deploy its Care Enablement platform with an existing provider organization in Hawai'i.

This initiative will integrate Elation’s EHR technology with Astrana’s value-based care infrastructure and care management capabilities.

The program will support over 100 primary care providers serving more than 20,000 Medicare patients in Hawai'i, with the aim of onboarding providers into Astrana’s Care Enablement platform by the end of Q3 2024.
