agilon health has expanded its partnership with Springfield Clinic in Illinois, with the healthcare provider joining one of agilon’s Senior Health Connect Accountable Care Organisations (ACOs).

Springfield Clinic is a community-based primary and specialty care organisation with more than 650 physicians and advanced practitioners across more than 80 specialties and 20 counties. The organisation began its long-term partnership with agilon in 2024 to transition to a full-risk, value-based care model, which currently supports around 18,000 senior Medicare Advantage patients.

Through the expanded partnership, the organisations expect to support approximately 21,000 additional Traditional Medicare beneficiaries across the communities served by Springfield Clinic.

Springfield Clinic will participate in agilon’s ACO under the Medicare Shared Savings Programme (MSSP). The model is designed to support more coordinated and personalised care, with a focus on prevention, earlier intervention and improved patient outcomes while reducing avoidable healthcare costs.

According to agilon, its ACOs have generated more than $510 million in gross savings since 2021, including $125 million in savings for the Medicare Trust Fund. In 2024, agilon’s ACOs achieved an average quality score of 96%. Four ACOs achieved perfect quality scores, while five were recognised as high performers across all quality measures.

Founded in 2016, agilon works with more than 30 physician groups and health systems to support full-risk, value-based care for Medicare Advantage and fee-for-service healthcare models.

The company uses technology and AI-enabled platforms to analyse healthcare data and provide physicians with insights to identify patient needs, support earlier interventions and address gaps in care. Its clinical pathways cover chronic conditions including heart failure, chronic obstructive pulmonary disease and dementia.

Springfield Clinic established its ACO in 2015 and has developed a physician-led approach to value-based care. Joining Senior Health Connect ACO will provide additional technology, analytics and operational support while allowing the organisation to maintain local clinical decision-making.

The expanded partnership is intended to strengthen coordinated care for older adults and support Springfield Clinic’s transition to broader accountable care. agilon will continue working with its physician partners to expand value-based care and improve outcomes for Medicare patients.
